How to register with us
- Please fill out a patient registration form and complete a new patient questionnaire which is available from reception or you can print one off below
- You can request to see a specific GP by asking the receptionist but you may have to wait longer to see the GP of your choice
- Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this)
- Your medical records may take several weeks to arrive at the Practice
- There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

Due to COVID-19 and limiting the footfall in the practice, please fill out our online registration form and email it back to us. If you are unable to do this please contact the surgery for advice on how to register. Please also complete ALL other forms.
